- Visit the Beneficiary Portal of Ayushman Bharat.
- Click “Beneficiary” and enter your registered mobile number.
- Enter the captcha → select “Generate OTP” → login using OTP.
- Choose your state and district, then check eligibility using your Aadhaar number.
- If your Ayushman Card is Not Generated, click “Apply Now.”
- Fill personal details, date of birth, family information, etc.
- Upload necessary documents like:
- Aadhaar Card
- Ration Card
- Residence proof
- Verify your identity using OTP sent to your Aadhaar-linked number.
- Review and submit your application.
Once approved, the card can be downloaded and used at any empanelled hospital.
