The Government of India’s flagship health insurance programme, Ayushman Bharat-Pradhan Mantri Jan Arogya Yojana (PM-JAY), continues to expand its reach, offering cashless medical treatment of up to ₹5 lakh per year to eligible low-income families.
In Uttar Pradesh, the state government launched a month-long Ayushman Card campaign on November 25, 2025, enabling citizens to apply for their health cards from home or through special camps set up in every district.


What Is Ayushman Bharat Yojana?

Introduced in 2018, PM-JAY aims to ensure accessible healthcare for economically weaker sections of society. The Ayushman Card acts as a health ID, allowing beneficiaries to receive free treatment across empanelled hospitals nationwide.

So far, over 34.7 crore Ayushman Cards have been issued to eligible individuals across India.

With this card, beneficiaries can avail:

  • Cashless treatment up to ₹5 lakh annually
  • Coverage for all family members
  • Treatment at both government and private empanelled hospitals
  • Financial support for major diseases and surgeries

How to Apply for Ayushman Card Online

Eligible citizens can apply for the Ayushman Card from home through the official portal. Here’s a step-by-step process:

  1. Visit the Beneficiary Portal of Ayushman Bharat.
  2. Click “Beneficiary” and enter your registered mobile number.
  3. Enter the captcha → select “Generate OTP” → login using OTP.
  4. Choose your state and district, then check eligibility using your Aadhaar number.
  5. If your Ayushman Card is Not Generated, click “Apply Now.”
  6. Fill personal details, date of birth, family information, etc.
  7. Upload necessary documents like:
    • Aadhaar Card
    • Ration Card
    • Residence proof
  8. Verify your identity using OTP sent to your Aadhaar-linked number.
  9. Review and submit your application.

Once approved, the card can be downloaded and used at any empanelled hospital.
